About the Role

Key Responsibilities

* Initiate and implement personalized physical therapy programs based on physician orders

* Maintain detailed evaluations, progress notes, and discharge summaries

* Collaborate with nursing and rehab staff to enhance residents' functional outcomes

* Contribute to interdisciplinary care planning meetings

* Maintain treatment documentation, track patient statistics, and request necessary supplies

* Participate in or lead staff training and education

* Provide leadership support in the absence of the Director of Physical Therapy

What We're Looking For

* Graduate of an accredited Physical Therapy program

* **Active NY State Physical Therapy license**

* Strong interest in working with **geriatric, subacute, and adult patient populations**

* Previous experience preferred (but not required---**new grads are encouraged to apply!**)

* Leadership qualities and ability to direct support staff when needed
