Description
A Registered Nurse (RN) or Licensed Practical Nurse (LPN) provides direct care to residents and is a critical part of the healthcare team. Working as part of a team, you will assess, plan, implement, and evaluate resident care plans to promote optimal health, safety, and well-being.
**Schedule:**PRN, Weekdays, Weekends, 12 Hour Shifts
***Primary Responsibilities and Duties:***
* Conduct comprehensive assessments of residents' health status, including vital signs, medical history, and current symptoms.
* Collaborate with residents, families, physicians, and other team members to establish goals of care and develop appropriate care strategies.
* Regularly reassess resident needs and adjust care plans accordingly, ensuring continuity and responsiveness to changing conditions.
* Provide skilled nursing interventions and treatments in accordance with physician orders and established care protocols.
* Administer medications, perform wound care, monitor vital signs, and manage medical equipment as needed to support resident health and recovery.
* Coordinate with ancillary services, including therapy, dietary, and social services to ensure comprehensive and integrated care delivery.
* Educate residents and families on health conditions, treatment options, medication management, and self-care strategies to promote understanding and empowerment.
* Provide emotional support, counseling, and advocacy to residents and families facing health challenges, fostering a trust and therapeutic relationship.
* Promote preventive health practices and wellness initiatives to optimize resident health and quality of life.
* Document nursing assessments, interventions, and outcomes in resident medical records.
* Participate in care conferences and team meetings to discuss residents' progress goals, and any changes in condition or care needs.
* Adhere to infection control protocols and maintain a clean, safe, and hygienic environment for residents, staff and visitors.
* Collaborate with other healthcare professionals, including registered nurses, physicians, therapists, and social workers, to optimize resident outcomes and promote team work.
* Perform all other duties assigned.
Requirements
***Job Qualifications:***
* Valid license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state where work is performed.
* Strong clinical skills and knowledge of nursing principles, procedures, and best practices in long-term care.
* Excellent communication and interpersonal skills, with the ability to interact effectively with residents, families, caregivers, and healthcare professionals.
* Demonstrated ability to prioritize tasks, manage multiple responsibilities, and adapt to changing priorities in a fast-paced environment.
* Understanding of regulatory requirements governing nursing practices and skilled nursing facilities.
* Ability to build rapport with patients through a compassionate and caring demeanor.
***Physical Requirements:***
* Work performed in a medical setting due to the need to work directly with residents.
* Must be able to sit, stand, bend, reach, talk, hear, stoop, kneel, crouch, stretch, and twist.
* Stamina to stand and move about the facility to perform frequent rounds and respond to emergent situations as needed.
* Emotional resilience and ability to cope with the physical and emotional demands of working with residents who may have complex healthcare needs.
* Capability to lift and transfer residents safely, with proper body mechanics and assistance as needed.
* Required vision includes close, vision, distance, peripheral, and the ability to adjust focus.
* Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
* The job requires accepting criticism and dealing calmly and effectively with high-stress situations.
* The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.
* The ability to keep your hand and arm steady while moving your arm or while holding your arm and hand in one position.
* The ability to exert maximum muscle force to lift, push, pull, or carry objects.
