Responsibilities
- Plan, implement, and adjust physical therapy interventions to restore functional abilities and reduce pain as part of a structured treatment program.
- Conduct initial assessments, set measurable goals, and monitor patient progress for assigned cases.
- Deliver individual and group therapy sessions focusing on mobility, strength, balance, and functional activities.
- Collaborate with an interdisciplinary team to coordinate care, discharge planning, and safety education.
- Document patient progress, participate in care conferences, and communicate findings with the care team.
- Provide patient and family education on home exercise programs and safety considerations.
